Terms and Conditions for Unlock Performance and Rehab

1. Acceptance of Terms By accessing the website of Unlock Performance and Rehab ("the Service"), you agree to be bound by these Terms and Conditions. If you do not agree, strictly do not use this website.
2. Medical Disclaimer (No Provider-Patient Relationship)
Educational Purpose Only: The content on this website (including text, videos, and blog posts) is for informational purposes only and is not medical advice, diagnosis, or treatment.
No Relationship: Use of this website or contacting us via email does not establish a physical therapist-patient relationship. A provider-patient relationship is only established after you have signed our specific Consent to Treat forms and completed an initial evaluation.
Consult Your Physician: Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition.
3. Telehealth & Jurisdiction Restrictions
Washington State Only: Unlock Performance and Rehab is licensed to practice Physical Therapy in the State of Washington. By scheduling an appointment, you certify that you will be physically located in Washington State at the time of your telehealth session.
Emergency Situations: We are not an emergency service. If you are experiencing a medical emergency, severe pain, or a life-threatening situation, call 911 immediately or go to your nearest emergency room.
4. Eligibility and Medicare Exclusion
Age Restriction: You must be at least 18 years old to use our services. We do not treat minors.
Medicare Exclusion: We are a private-pay practice and are not enrolled with Medicare. By using our services, you certify that you are not a Medicare beneficiary (generally age 65 or older, or collecting SSDI). We cannot provide physical therapy services to Medicare beneficiaries at this time, even on a cash-pay basis, due to federal regulations.
5. Financial Policy (Cash-Based Services)
No Insurance Billing: We are an "out-of-network" provider. We do not bill insurance plans. Payment is due in full at the time of service.
Payment Methods: We accept major credit cards (via Stripe/Square) and Venmo.
Superbills: We do not provide a "Superbill" for you to submit to your insurance for potential out-of-network reimbursement.
6. Cancellation and No-Show Policy Your appointment time is reserved specifically for you.
24-Hour Notice: You must provide at least 24 hours' notice to cancel or reschedule an appointment.
Fees: Cancellations made with less than 24 hours' notice, or failure to attend a scheduled appointment ("No-Show"), will result in a charge of the full session fee of $80. This fee must be paid before future services are rendered.
7. Intellectual Property All content on this website, including the "Unlock Performance and Rehab" name, logo, and original text/videos, is the property of Unlock Performance and Rehab and is protected by copyright laws.
8. Limitation of Liability To the fullest extent permitted by Washington law, Unlock Performance and Rehab shall not be liable for any indirect, incidental, or consequential damages resulting from your use of this website or reliance on any information provided herein.
9. Governing Law These Terms shall be governed by the laws of the State of Washington. Any disputes arising from these Terms shall be resolved in the courts of Chelan County, Washington.
